Fundamentals and Rec 1 Instructor Intern Fees

BEFORE SUBMITTING THIS FEE, YOU MUST:

1. Provide GUE HQ with proof of an annual medical exam that documents that you are found suitable for diving in line with procedures delineated by a competent medical authority. If such procedures are not specified, you must provide evidence that a medical examination not older than one year has been conducted and that the you are free of any health concerns and approved for diving. GUE HQ has a form available for instructors to provide their doctors for guidance, here: Sample Diver Physical
   Provided documentation must specifically indicate approval for diving, and if using the provided sample form, a physician's signature is required on page 3.
   
2. Meet GUE's Instructor Fitness Requirements and submit a completed Fitness Form to GUE HQ. Details about the requirements and the form can be downloaded from the following links:

3. Read and sign the GUE Instructor Candidate Agreement: Instructor Candidate Agreement
   
4. Complete the Fundamentals Instructor Application Signature Form, or Rec 1 Instructor Application Signature Form if selecting Rec 1-only registration, and return it to HQ.
   
5. Meet with an Instructor Trainer or Evaluator to complete the Instructor Candidate Assessment Form. If you are not yet in contact with an IT or IE, please contact HQ for support.
